IV. Enrollment and case closure process
Here is the full process from receiving a request to closing a case. The labels show the time limit for each stage.
1
Receive an enrollment request
- Sources: discharge planning referral, Long-Term Care Management Center referral, or an application from the patient or family.
- The home nurse will phone the family within 72 hours.
▼
2
Eligibility assessment
- Following the NHI Administration's home nursing enrollment guidelines, we assess whether the patient meets the criteria and which stage of care applies.
- We explain the services and fee rules to the family.
Not eligible → we reply to the family and the referring unit. Eligible → move on to the next step.
▼
3
Schedule the visit
- We contact the main caregiver to confirm available visit times and required documents.
- We arrange the doctor's visit time.
▼
4
First visit and care
- During the visit, medical records are logged with the NHI card and uploaded to the insurer for the record within 24 hours.
- We complete the assessment and draw up a care plan: assessing the patient's physical condition, medications, and care needs, treating wounds, changing tubes, and providing the care and guidance the family needs.
▼
5
Sign the contract and information sheet
- We explain the rights and responsibilities of both sides to the patient or family and ask them to sign the Home Nursing Station Care Contract and the Medication Integration Information Sheet.
- We leave the home nurse's business card so the patient can get in touch.
▼
6
Payment
- Fees are calculated by the services provided, plus the transportation fare, and collected from the family or patient. We let them know the receipt will be handed over at the next visit.
▼
7
Complete NHI enrollment
- Within 2 weeks of enrollment, we register the home care case for claim processing (National Health Insurance home care application form, via VPN).
- Create the patient file.
▼
8
Records
- We enter the home visit records (whole-person assessment form, care plan, and evaluation).
- Medical records are completed within 3 days of the visit, filed in the patient's own folder, and kept in a locked records cabinet.
▼
9
Follow-up visits
- Follow-up visits are arranged based on the doctor's orders, the care plan, and the patient's needs.
- When needed, we connect you with other professionals (pharmacists, respiratory therapists, social workers, psychologists, and more).
▼
10
Is extended care needed?
- If extension is needed: apply within 30 days before the care period ends.
Extension needed → return to “Follow-up visits” and continue care. No extension needed → proceed to case closure.
▼
11
Case closure
- A case is closed in the event of death, moving away, a hospital stay of more than 30 days, improved health, refusal of visits, or danger to the home nurse's safety.
- We follow up by phone with the patient or family and record the outcome in the closing medical summary.
- Patient records are kept for 7 years.
