1. About the service

Map of the six long-term care transport districts in Chiayi County

Chung Cheng Gaoling long-term care transportation covers five of the six districts of Chiayi County: District 1 (Dongshi, Budai, Puzi, Yizhu), District 2 (Shuishang, Taibao, Lucao), District 3 (Xingang, Liujiao, Xikou), District 4 (Dalin, Minxiong, Alishan) and District 5 (Zhuqi, Meishan). That is 15 towns and townships in all, linking a full care network from the coast to the plains and the mountains. District 6 (Zhongpu, Fanlu, Dapu) is not served yet, and we will extend coverage step by step as our resources grow. Wherever older adults live in Chiayi County, we aim to offer transportation that is visible, easy to reach and affordable, so that every older adult in need can go out safely and come home with peace of mind.

Under the Ministry of Health and Welfare's long-term care rules, transportation services fall into two types, based on the destination and the budget they draw on:

A. Transportation

Code: DA01

Scope: rides for people who need long-term care between home and medical facilities.

Purpose: rides from home to a medical facility and back for medical visits, regular rehabilitation or dialysis.

B. Community service transportation

Code: BD03

Scope: rides for people who need long-term care between home and community long-term care sites.

Purpose: rides from home to community-based care providers (day care centers, small multi-function centers, home-based day care), neighborhood care stations, cultural health stations, community dementia care sites, assistive device centers or day care services for people with disabilities, and back.

2. Shuttle information

Below are the current locations of our shuttle buses, with one map window per vehicle, refreshed automatically about every 15 seconds. Locations are only a reference for passengers and families to follow the vehicles, and actual arrival times depend on traffic. To book or ask questions, call 05-220-1001 or book online now.

The dots on the map are vehicles, and the number inside each dot is the vehicle number. Red = in motion, yellow = stopped, gray = offline (no report for over 15 minutes, so the last known position is shown). Click the map once with your mouse to zoom with the scroll wheel. On a phone, use the + / - buttons on the map or pinch with two fingers.

3. How to apply and use the service

1

Apply for long-term care

Call the Long-Term Care Hotline 1966, or apply online through your county or city Long-Term Care Management Center website.

The government will send a care manager to your home to assess you. Your disability level must be approved at Level 2 to 8.

2

Make a care plan

Once you are approved, a case manager will talk through your needs with your family and add ‘Transportation (DA01)’ or ‘Community transportation (BD03)’ to the care plan.

3

Find and contact a contracted provider

Your case manager will give you a list of contracted long-term care transport providers nearby. Chiayi County Government also runs an official transport booking platform (a dedicated app), where family members can sign up and link the care recipient's information.

4

Book your ride in advance

Please book at least 3 days ahead (book online now). Service hours are Monday to Friday, 8:00-12:00 and 13:00-17:00.

As a rule, we cannot take last-minute requests unless a vehicle is free that day. When booking, please give the exact pick-up time, the start and end points (such as the hospital), and whether you need wheelchair access.
