An A-Unit (Community Integrated Service Center) is like your personal long-term care planner. Our team of professional case managers visits your home after your county or city Long-Term Care Management Center has assessed you. We design a care plan that fits the older adult's condition and your family's needs, and we connect you with the government long-term care resources you can use, to ease the care load on your whole family.
You can apply if you live in Chiayi County, have been assessed at long-term care need level (CMS) 2 to 8, and meet any one of the following conditions:
| Group | Eligibility | 💡 What is new in Long-Term Care 3.0 |
|---|---|---|
| Older adults with disabilities | Adults aged 65 and over who need help with daily activities | Restrictions on households with a live-in migrant caregiver have been lifted, so they can also apply for many community care and rehabilitation services. |
| People with dementia | People of any age diagnosed with dementia by a doctor | It used to be limited to people aged 50 and over, and coverage is now fully expanded. |
| People with disabilities | People of any age holding a disability certificate | Stronger links with hospitals for post-acute care (PAC). |
| Indigenous people with disabilities | Indigenous people aged 55 and over with disabilities | They receive a higher share of subsidies for transport and services in remote areas. |
| Frail older adults | Older adults whose intrinsic capacity is declining and who are at risk of frailty but not yet disabled | Newly included in Long-Term Care 3.0, with a focus on preventing and delaying disability. |
We provide the most important case management services. Both core packages are fully funded by the government, with no co-payment for you:
Review every six months (AA01): while following up on services, the case manager reviews the care plan every six months and adjusts services as needed, so the plan keeps meeting the older adult's needs.
Possible next steps:
- If the provider has capacity → it contacts the client or family to sign the contract. If it has no capacity → it asks another provider for help (and files a change report and a new referral).
- If service can start within 4 days → service begins on the planned date. If it cannot start within 4 days → the family changes the service date, or the client has passed away or declines service (the case is closed after a change report).

To protect your rights, Chung Cheng Gaoling follows a standard process that keeps the privacy of clients and families strictly confidential:
After the complaints officer receives a report, they log and sort the case, then accept it and look into the issue.
The unit supervisor steps in and investigates the matter in depth.
The people involved and the organization arrange talks and work actively toward an agreement.
Processing time: general complaints are handled within 7 days (special circumstances excepted). Reply method: the complainant is told the outcome verbally or in writing. Administrative reporting: the director is informed at the same time, and the competent authority is notified (a change report or official letter is filed as the case requires).
Follow-up and filing: once the improvement measures are confirmed, the case is tracked and filed so that service quality keeps getting better.
If you feel our response is still incomplete, you can also contact the government authorities directly:
Chiayi County Public Health Bureau, tel. 05-3620600
Long-Term Care Hotline: dial 1966 (free for the first 5 minutes)
If a case needs further discussion, the staff and the facility concerned will arrange talks. Once an agreement is reached, it is reported to the director, who then replies to the complainant with the outcome.
